After user clicks on the Add button, the following popup appears as shown in the image.

Facility: Entet the name of the facility under this field.

External Facility ID: Enter the external faiclity ID under this field.

Notes: User can add the notes under this field

Status: Select Active radio button to make the facility active (or) Inactive radio button to make the facility inactive.

Save: Click on ‘Save’ button, after adding the details of the new facility.

·Close: Click on close button to close the popup window.
